Frequently asked questions about Anorectal Surgery

Are piles and hemorrhoids the same thing?
Yes. Piles is the common Indian/British English term for hemorrhoids — swollen vascular cushions inside or around the anal canal. They are graded 1 to 4 based on severity. Grade 1–2 often respond to diet and medication; Grade 3–4 typically require a procedure.
How do I know if I need surgery for piles?
Surgery is recommended when piles bleed persistently despite dietary changes, when they prolapse (come outside) and cannot be pushed back, when they cause constant discomfort or soiling, or when medication and rubber-band ligation have not worked. Grade 3 and 4 hemorrhoids almost always need a surgical procedure.
Is anorectal surgery painful?
Modern techniques — particularly laser and LigaSure — are significantly less painful than older open hemorrhoidectomy. Most patients manage with oral pain medication alone. The procedure itself is performed under anaesthesia. Pain after laser or stapler procedures is typically described as mild to moderate — comparable to a sore throat — rather than severe.
What is the difference between piles, fissure and fistula?
Piles (hemorrhoids) are enlarged blood vessels around the anal canal, causing bleeding, prolapse and discomfort. An anal fissure is a tear in the skin of the anal canal, causing sharp pain during and after passing stools. An anal fistula is an abnormal tunnel connecting the inside of the anal canal to skin outside the anus, causing discharge and repeated abscess formation. These are three distinct conditions requiring different treatments.
Can I eat normally before and after surgery?
Before surgery: light diet the day before; fasting from midnight for morning procedures. After surgery: all procedures allow a normal soft diet from day 1 — dal, khichdi, curd, soft roti. High-fibre foods (vegetables, fruits, whole grains) are actively encouraged from day 3 to prevent constipation. Avoid very spicy food for 1–2 weeks.
