What is stapler piles surgery (PPH)?

The Procedure for Prolapse and Hemorrhoids (PPH), also called stapler hemorrhoidopexy or MIPH (Minimally Invasive Procedure for Hemorrhoids), uses a circular stapling device to remove a ring of mucosa and submucosa above the hemorrhoids, lifting them back into their normal anatomical position and interrupting their blood supply simultaneously.

The key advantage of PPH over conventional hemorrhoidectomy is that the entire procedure happens above the sensitive pain-bearing anal skin — in the insensitive rectal mucosa above the dentate line. This results in dramatically less postoperative pain and a much faster recovery.

PPH is particularly effective for Grade 3 prolapsing hemorrhoids and Grade 4 hemorrhoids with internal component. It is also the preferred technique for circumferential hemorrhoids (haemorrhoids at multiple positions that would be difficult to excise individually without causing stenosis).

How it's done

The procedure, step by step

Anaesthesia

Spinal or general anaesthesia. Procedure takes 25–35 minutes total.

Dilator insertion

A circular anal dilator (CAD) is gently inserted and fixed in place with sutures to the perianal skin. This provides a clear, atraumatic working channel.

Purse-string suture

A Prolene purse-string suture is placed circumferentially in the rectal mucosa 3–4 cm above the dentate line, incorporating the prolapsed mucosa.

Stapler insertion

The PPH circular stapler is introduced through the dilator, opened, and its anvil is advanced above the purse-string suture. The purse-string is tied, drawing the prolapsed mucosa into the stapler.

Firing the stapler

The stapler is closed and fired — simultaneously excising a ring of hemorrhoidal tissue and stapling the cut edges together. The staple line is inspected for completeness and haemostasis.

Discharge

Pack removed after 1–2 hours. Normal diet from evening. Discharged next morning with stool softeners and mild analgesia.

Is stapler surgery better than laser for piles?
Neither is universally better — each has an optimal application. Laser hemorrhoidoplasty is best for Grade 1–3 hemorrhoids with internal component only; it is minimally invasive and recovery is slightly faster. Stapler PPH is better for Grade 3–4 prolapsing or circumferential hemorrhoids — it lifts all hemorrhoids simultaneously and is particularly effective when prolapse is the dominant symptom. Dr Samir Contractor will recommend the appropriate technique based on your grade, anatomy and symptoms.
Does stapler piles surgery leave a scar?
No visible external scar. The staple line is entirely inside the rectum, above the dentate line. Externally, there are no incisions or sutures. Many patients are surprised to find no wound to dress or care for after discharge.
What are the risks of stapler PPH?
PPH is a safe, well-established procedure. The main risks are: bleeding (1–2%, usually minor), recurrence of prolapse (3–5% at 5 years with appropriate patient selection), urgency or frequency of defecation in the first few weeks (usually resolves), and rarely, rectovaginal fistula or rectal stenosis (< 0.1% in experienced hands). The risk of serious complications is significantly lower than open hemorrhoidectomy.
I have external skin tags along with piles — will PPH treat them?
PPH addresses internal and prolapsing hemorrhoids but does not remove external hemorrhoidal tags or folds. If external tags are significant, a combined procedure (PPH plus trimming of external tags) may be appropriate, or a LigaSure hemorrhoidectomy which treats both components. This is assessed at your consultation.
Can I have PPH if I've had previous piles surgery?
Previous conventional hemorrhoidectomy is not an absolute contraindication, but it may make PPH more technically challenging depending on the anatomy. Previous stapler PPH generally requires a different approach for recurrence. Dr Samir will assess your prior surgical history and recommend the best option.
