The Procedure for Prolapse and Hemorrhoids (PPH), also called stapler hemorrhoidopexy or MIPH (Minimally Invasive Procedure for Hemorrhoids), uses a circular stapling device to remove a ring of mucosa and submucosa above the hemorrhoids, lifting them back into their normal anatomical position and interrupting their blood supply simultaneously.
The key advantage of PPH over conventional hemorrhoidectomy is that the entire procedure happens above the sensitive pain-bearing anal skin — in the insensitive rectal mucosa above the dentate line. This results in dramatically less postoperative pain and a much faster recovery.
PPH is particularly effective for Grade 3 prolapsing hemorrhoids and Grade 4 hemorrhoids with internal component. It is also the preferred technique for circumferential hemorrhoids (haemorrhoids at multiple positions that would be difficult to excise individually without causing stenosis).

A circular anal dilator (CAD) is gently inserted and fixed in place with sutures to the perianal skin. This provides a clear, atraumatic working channel.
A Prolene purse-string suture is placed circumferentially in the rectal mucosa 3–4 cm above the dentate line, incorporating the prolapsed mucosa.
The PPH circular stapler is introduced through the dilator, opened, and its anvil is advanced above the purse-string suture. The purse-string is tied, drawing the prolapsed mucosa into the stapler.
The stapler is closed and fired — simultaneously excising a ring of hemorrhoidal tissue and stapling the cut edges together. The staple line is inspected for completeness and haemostasis.
Pack removed after 1–2 hours. Normal diet from evening. Discharged next morning with stool softeners and mild analgesia.
Fluids within a few hours. Mild anal pressure — not acute pain in most patients. Stool softener started immediately to prevent constipation.
Most patients discharged on day 1. Normal soft diet — dal, khichdi, soft roti, curd, fruits. Mild oral pain relief sufficient.
Light walking. Warm sitz baths 2–3 times daily. Continue stool softener. Avoid prolonged sitting.
