What is LigaSure hemorrhoidectomy?

LigaSure hemorrhoidectomy uses an advanced bipolar energy sealing device (the LigaSure vessel sealer) to simultaneously cut and seal the hemorrhoidal tissue and blood vessels. Unlike conventional open hemorrhoidectomy where raw wounds are left to heal, LigaSure seals the tissue as it cuts — resulting in significantly less bleeding during surgery and considerably less postoperative pain.

LigaSure hemorrhoidectomy is the preferred surgical option for large Grade 3 and Grade 4 hemorrhoids where laser treatment would be insufficient. It excises the hemorrhoid completely and permanently — recurrence rates are very low at under 2% at 5 years.

The procedure, step by step

Anaesthesia and positioning

General or spinal anaesthesia. Lithotomy position. Proctoscopy to assess hemorrhoids and plan excision.

Hemorrhoid exposure

Each hemorrhoidal cushion is grasped and placed on gentle traction to expose its base and pedicle.

LigaSure sealing and excision

The LigaSure device is applied at the hemorrhoid pedicle. A single activation seals and cuts the blood supply and hemorrhoidal tissue simultaneously. The sealed pedicle is safe, dry and requires no suture ligation.

Wound management

LigaSure leaves a small, sealed wound rather than the large raw area of conventional hemorrhoidectomy. The wound may be left open or partially closed depending on anatomy. A small gauze pack is placed.

Three columns treated

Typically three hemorrhoidal columns (at 3, 7, and 11 o'clock positions) are treated in sequence, preserving adequate skin bridges between wounds to prevent stenosis.

Day 0–1

What is the difference between LigaSure and laser piles treatment?
Laser hemorrhoidoplasty (laser piles) shrinks the hemorrhoid from inside without excising it — best for Grade 1–3. LigaSure hemorrhoidectomy completely removes the hemorrhoid tissue using an energy-sealing device — better for large Grade 3 or Grade 4 hemorrhoids. Laser is less painful and quicker to recover from; LigaSure is more definitive for large prolapsing piles.
Is LigaSure hemorrhoidectomy painful?
It is significantly less painful than conventional (Milligan-Morgan) open hemorrhoidectomy because the LigaSure device seals the wound as it cuts, reducing the raw wound area. However, it is more painful than laser or stapler procedures. Most patients manage well with oral paracetamol and NSAIDs, and stool softeners are essential to prevent painful bowel movements in the first week.
Can all three piles be removed at once?
Yes. Typically all three hemorrhoidal columns (primary hemorrhoids at 3, 7, and 11 o'clock positions) are removed in a single operation. The key technical principle is preserving adequate skin bridges between the excision sites to prevent the rare complication of anal stenosis.
How long is the hospital stay?
24–48 hours is typical. You are admitted the day of surgery, and most patients are comfortable enough for discharge the following morning. Patients who live far from Vadodara or have no assistance at home may stay the second night.
What are the risks of LigaSure hemorrhoidectomy?
Primary bleeding (within 24 hours) is rare as the LigaSure device seals vessels effectively. Secondary bleeding at 7–10 days occurs in approximately 1–2% of cases and is usually manageable.
