What is laser piles surgery?

Laser hemorrhoidoplasty (LHP) uses a laser fibre inserted into the hemorrhoidal tissue to deliver precise laser energy, causing the hemorrhoidal blood vessels to shrink and the supporting tissue to tighten. Unlike conventional surgery, there is no cutting of the anal skin, no external wound, and no sutures — making it significantly more comfortable than traditional open hemorrhoidectomy.

The procedure takes approximately 30 minutes under short general or spinal anaesthesia. You are discharged the same day and can walk home, eat a normal meal, and resume light activities the same evening.

Laser treatment is most effective for Grade 1, 2 and early Grade 3 hemorrhoids. For large prolapsing Grade 3 or Grade 4 hemorrhoids, a stapler (PPH) or LigaSure approach may be more appropriate — this is assessed at your consultation.

The procedure, step by step

Anaesthesia

Short general anaesthesia or spinal anaesthesia (30–45 minutes total). You are completely comfortable throughout.

Positioning and proctoscopy

You are positioned in the lithotomy position. A proctoscope is gently inserted to visualise the hemorrhoids and identify their feeding vessels (the submucosal arteries).

Doppler identification

A Doppler probe may be used to precisely locate the terminal branches of the superior hemorrhoidal artery that supply the hemorrhoidal cushions — ensuring accurate laser delivery.

Laser fibre insertion

A thin laser fibre (980 nm diode laser) is inserted directly into the hemorrhoidal tissue through the proctoscope. No skin incision is needed.

Laser energy delivery

Laser energy is applied in controlled pulses, coagulating the hemorrhoidal blood vessels and causing the swollen tissue to shrink. The fibre is withdrawn in a controlled manner, treating the entire hemorrhoid.

Recovery and discharge

You are moved to recovery for 60–90 minutes, then to the day-care ward. Once fully awake and comfortable, you are discharged — typically within 3–4 hours of the procedure.

Day 0

Procedure day

Discharged 3–4 hours post-procedure. Mild fullness or discomfort in the anal area is normal. Normal soft diet the same evening — dal, khichdi, curd.

Day 1–2

Rest at home

Light walking encouraged. Mild soreness during bowel movements is common and managed with stool softener (Duphalac / Cremaffin) and paracetamol. Normal Indian diet — avoid very spicy food.

Day 3

Return to desk work

Most office workers and teachers return to work. Driving is fine if comfortable. Avoid prolonged sitting on hard surfaces.

Week 1–2

Full light activity

Light gym, walking, cycling. All foods tolerated. Sitz baths (warm water soaks) 2–3 times daily speed healing.

Week 3–4

Normal activity

Full physical activity resumed. Final check-up with Dr Samir at 4 weeks. Continue high-fibre diet long-term to prevent recurrence.

Is laser piles surgery painful?
Laser hemorrhoidoplasty is one of the least painful surgical treatments for piles available today. The procedure itself is performed under anaesthesia, so you feel nothing. After surgery, most patients experience mild discomfort — particularly during the first bowel movement — which is managed effectively with stool softeners and oral paracetamol. The absence of an external wound is the key reason laser surgery is so much more comfortable than open hemorrhoidectomy.
How long does the laser piles procedure take?
The actual laser treatment takes 15–30 minutes. Including anaesthesia, positioning and recovery, you are typically in the hospital for 4–6 hours total. The procedure is performed on a day-care basis — you are admitted in the morning and discharged by afternoon.
Will my piles come back after laser surgery?
Laser hemorrhoidoplasty has a recurrence rate of under 5% at 2 years when combined with dietary changes — increasing fibre intake (fruits, vegetables, whole grains), drinking 2–3 litres of water daily, and avoiding prolonged straining. Most recurrences are attributable to persistent constipation or dietary habits rather than the procedure itself.
How many sittings of laser treatment are needed?
Typically one sitting is sufficient. Depending on the number and grade of hemorrhoids, occasionally 2–3 hemorrhoidal columns are treated in the same session. It is unusual to need more than one procedure.
What grade of piles is treated with laser?
Laser hemorrhoidoplasty is most effective for Grade 1 (small, internal, no prolapse), Grade 2 (prolapse with straining but reduces spontaneously), and early Grade 3 (requires manual reduction after prolapse). For Grade 4 (fixed prolapse that cannot be reduced) or large Grade 3 hemorrhoids, a stapler PPH or LigaSure hemorrhoidectomy gives more reliable long-term results. Your grade is assessed by proctoscopy at consultation.
What diet should I follow after laser piles surgery?
Day 1–3: soft, non-spicy food — dal, khichdi, curd, banana, soft-cooked vegetables, buttermilk. Week 1–2: regular Indian diet, avoiding very spicy curries and excessive chilli. Week 2 onwards: actively increase fibre — add green leafy vegetables, fruits, whole wheat roti, oats, and lentils. Long-term: 2–3 litres of water daily and a high-fibre diet are the most important factors in preventing recurrence.
